    I give you a lot of credit for attempting to devise a survey. However, you fall short of presenting a questionnaire that is understandable to everyone attempting to respond. Specifically,

    You will not get comparable data to tablize because each response will answer differently.
    Read up on research methods. Get a book at the library. Revise and get a better survey. Good luck.
